Sport and Spine Physical Therapy Patient Forms We will verify all physical therapy benefits for the patients in or out of network prior to their first visit and communicate the information to the patient. Sport and Spine is currently a provider for the following insurance companies:

If you do not see your health insurance company listed, please call us and we will verify the information for you. Please note, there are many out of network/PPO benefits that we can accept. In some cases, the out of network and in network benefits may be comparable.

Workman’s Compensation:
We accept all workmen’s compensations including self-insured companies.

Auto Insurances:
All auto insurances are accepted if the accident occurred before July 2004. Med Pay is accepted if the accident occurred after July 2004.
